FBC Ministry Event Planning Form

Please allow 1-2 weeks after submission before following up. This form is intended to be completed anytime you plan to use the church facility for an FBC ministry meeting or event and need assistance with promotions or require resources from the church staff. For any non-FBC-related events, please refer directly to the Non-Ministry Facility Request Form (https://aboutfbc.churchcenter.com/people/forms/268268).

Any tech needs or setup requests (equipment or personnel) must be submitted with adequate time in order for us to meet your needs. Any changes made after the deadline may not be able to be completed!
